Uneven shoulders happen when one shoulder sits noticeably higher or more forward than the other. The cause is rarely in the shoulder itself. Why is one shoulder higher than the other?

The most common reason is that something below the shoulders is out of alignment. Your body is very good at adapting. If the spine curves to one side, or the pelvis tilts, the shoulders adjust to keep your eyes and head level. That compensation is what creates the visible unevenness.

A sideways curve of the spine is one of the best-known reasons for one shoulder sitting higher than the other.

Pelvic tilt

If one side of the pelvis is higher, the spine shifts sideways and the shoulders follow.

Leg-length difference

Even a small difference can tip the pelvis, creating a chain reaction all the way up to the shoulders.

Muscle imbalance or habit

Years of carrying a bag on one side, sitting in a twisted posture, or favouring one hand can create lasting imbalance.


Can uneven shoulders be caused by something in your hips or legs?

Yes. Your body works as a connected chain from your feet to your head. If one link moves out of position, everything above and below adjusts. A tilted pelvis makes the spine curve. A curved spine makes the shoulders uneven. Uneven shoulders make the neck muscles work harder. This is called compensation, and it is why pain often shows up far from its true source.

How is shoulder alignment assessed when you are standing up?

It is assessed using an EOS standing scan, which captures your full skeleton while you stand in your natural posture. This matters because lying-down imaging, such as a standard X-ray, CT or MRI, removes the effect of gravity. Your muscles relax, your curves flatten out, and the alignment shift that causes your uneven shoulders may not show at all.

A standing EOS image shows every link in the chain at once. Your radiographer and clinician can trace the line from your feet through your knees, hips, pelvis and spine to your shoulders. They can see exactly where the compensation starts and how it travels upward.

The EOS system uses about 90% less radiation than a standard X-ray, around ten times less. It takes only a few seconds to capture the full image, and there is no enclosed tunnel.

When is MRI better? EOS shows bones, joints and alignment clearly. If your clinician suspects a soft-tissue problem, such as a rotator cuff tear, a disc issue, or nerve compression, MRI is usually the better tool. The two types of imaging often work well together.


What can help with uneven shoulders?

The right approach depends entirely on what is causing the unevenness. That is why accurate diagnosis comes first. Here are some of the directions a clinician may consider once the cause is clear.

  • Targeted physiotherapy. Strengthening weaker muscles and stretching tighter ones can gradually improve balance, especially when the issue is largely muscular.
  • Addressing a pelvic tilt or leg-length difference. If the root cause is lower down, a clinician might look at orthotics, manual therapy, or other approaches to level the foundation.
  • Posture awareness. Simple changes to workstation setup, bag-carrying habits, or sleeping position may reduce further strain.
  • Specialist referral. Where a spinal curve or structural issue is involved, your clinician may refer you to an orthopaedic or spinal specialist for further assessment.
